Adult/Elder Care Coordinator Posting

RESPONSIBILITIES:

Reporting to the Health Program Services Manager, the Adult/Elder Care Coordinator develops, implements and administers Adult/Elder Care and initiatives related to providing quality care specifically for Elders, adults with disabilities (19 years of age or older), individuals recuperating from an illness/injury or who are incapacitated within the Tsawout First Nation community. The Adult/Elder Care Coordinator liaises with home support services for the above mentioned client groups that include, but are not limited to, providing supplementary care services for Tsawout First Nation clientèle in an institutional setting (i.e., hospital, care facility).

These services can include transportation, shopping, and other light duties or minor home maintenance activities. Overall, the Adult/Elder Care Coordinator works closely with the Tsawout Home and Community Care Program Staff and other Adult/Elder Care agencies in promoting and maintaining the physical, psychological and social well-being of their Adult/Elder care clientèle.
